Assistance for People with a Chronic Disease
The Chronic Disease Fund is one of the few charitable organizations to provide direct and immediate financial assistance to underinsured patients with chronic disease, cancers or life-altering conditions who cannot afford the co-payments for the expensive medications they need.
Financial assistance is awarded on a calendar year basis to patients that have insurance and meet the income qualification guidelines. Each application for assistance is reviewed by a team taking into consideration income levels by geographic area, the number of household dependents and current out-of-pocket medical expenses. Patients may enroll in programs either directly through the Fund or through participating pharmacies. Patients are enrolled on a first-come, first-served basis.
