Assistance to Families Dealing with Childhood Cancer

Zacs LegacyZac’s Legacy Foundation, Inc. provides financial assistance to parents enduring the struggles of a dependent child while in treatment for cancer.

How to apply:
Contact your hospital social worker/ medical care provider.

Fill out the application provided by the social worker / medical care provider. If your social worker / medical care provider does not have the Zac’s Legacy Foundation application forms, have the hospital social worker / medical care provider call 1-970-330-9000 to get the proper forms.

Return the completed application to the social worker/ medical care provider who will then notify Zac’s Legacy Foundation of your application.

At this time, the Foundation can only help resident families in the state of Colorado . The dependent child must be in treatment for cancer.
