<p>How to apply:<br />
Contact your hospital social worker/ medical care provider.</p>
<p>Fill out the application provided by the social worker / medical care provider. If your social worker / medical care provider does not have the Zac&#8217;s Legacy Foundation application forms, have the hospital social worker / medical care provider call 1-970-330-9000 to get the proper forms.</p>
<p>Return the completed application to the social worker/ medical care provider who will then notify Zac&#8217;s Legacy Foundation of your application.</p>
<p>At this time, the Foundation can only help resident families in the state of Colorado . The dependent child must be in treatment for cancer.</p>

			<content:encoded><![CDATA[<p>I am an insulin dependent type 1 diabetic, diagnosed a year and a half ago.  I have a wife and three precious boys, 4, 2, and 1.  My wife was pregnant with our third when we found out I was a diabetic.  I am on a wonderful, but costly pump therapy (omnipod.)  In 2009 we were able to work with a company called American Diabetes Wholesale who accepted what the insurance paid for my supplies, as well as paid our deductible.  We received a phone call from them last week however, saying that they would no longer help us.  They didn&#8217;t make enough money from our case.  This news was devastating for us because we&#8217;re already doing the paycheck to paycheck thing.  We will now need to come up with about 500 dollars a month&#8230;we don&#8217;t have it.  We&#8217;re trying to find some other form of assistance; maybe you can help.  I&#8217;ve been an insurance salesman for thirteen years, but that gets me no break in cost.  We pay 733.00 monthly in premiums, and have a 3000 deductible.  I don&#8217;t know what else to say except that we&#8217;re simple people with a quiet life, just trying to make ends meet while living daily with this chronic, life altering, disease.  Thank you for any assistance you can offer.<br />

			<content:encoded><![CDATA[<p>EyeCare America<sup>®</sup> has a Senior Eye Care program.  This program ensures that every senior has access to medical eye care and promotes annual, dilated eye exams. It raises awareness about age-related eye disease, including cataracts, provides free eye care educational materials and facilitates access to eye care—at no out-of-pocket cost.</p>

<p>The Seniors EyeCare Program is designed for people who:</p>
<ul>
<li>Are US citizens or legal residents</li>
<li>Are age 65 and older</li>
<li>Have not seen an ophthalmologist in three or more years</li>
<li>Do not belong to an HMO or the VA</li>
</ul>
<p>People eligible for a referral through the program receive a comprehensive, medical eye exam and up to one year of care— at no out-of-pocket cost —for any disease diagnosed during the initial exam.</p>
<p>Volunteer ophthalmologists accept Medicare and/or other insurance reimbursement as payment in full; patients without insurance receive care at no charge.</p>

Each application for assistance is reviewed by a team taking into consideration income levels by geographic area, the number of household dependents and current out-of-pocket medical expenses. Patients may enroll in programs either directly through the Fund or through participating pharmacies.  Patients are enrolled on a first-come, first-served basis.</p>

			<content:encoded><![CDATA[<p>The Victim Compensation Program (CalVCP) can help pay unreimbursed expenses that result when a violent crime occurs. Victims of crime who have been injured or have been threatened with injury may be eligible for help from CalVCP. CalVCP may assist with the cost of medical and dental treatment, mental health services, funeral and burial expenses, home security, residential crime scene cleanup and emergency relocation.</p>

<p>If a person is disabled as a result of the crime, CalVCP also assists with wage loss, dependent support loss, job retraining and home or vehicle modifications.</p>
<p>CalVCP can help pay moving expenses – such as first and last month&#8217;s rent, deposits and temporary lodging – if a victim needs to relocate for their own safety or emotional well-being.</p>
<p><strong>Emergency Awards</strong></p>
<p>Victims can request an emergency award in certain situations. Emergency awards are made based on substantial hardship and the immediate need for payment. Emergency awards are most often obtained to cover relocation or funeral/burial expenses. You must indicate this on your application in the emergency award section.</p>

<p><strong>Grant Guidelines</strong></p>
<ul>
<li>Grant requests are accepted from, or on behalf of, individuals diagnosed with lupus who live in the greater Los Angeles area.</li>
<li>Applicants must offer satisfactory evidence that lupus has been medically diagnosed. Evidence provided must include a doctor’s verification (a medical release form).</li>
<li>Applicants must demonstrate a significant financial need and show how the grant will either meet the need in full or supplement other resources.</li>
<li>Emergency Grants are paid by check directly to the agency or provider.</li>
<li>Grants are limited to 2 per person in any 12-month period, with an annual total of no more than $500.</li>
<li>Grants are not considered for funding when assistance from other sources (family, government, social agency, etc.) is deemed available.</li>
